Tuesdays, Wednesdays, Thursdays, Fridays & Saturdays.

(T-F: 9 am - 8 pm; Sat.: 10 am - 2 pm).

Client Paperwork

To become a client/patient - please call me first at 772-426-9955 to establish (I may not have availability, so best not to fill out paperwork first prior to knowing if I can take you on). You will then be directed by email to create a portal account. The portal is where you will be instructed on what paperwork is needed and the procedures pertaining to our meeting.
